There are no minimum income requirements or forms to fill out to participate. You just order online and pickup on a designated weekend at a local church that participates. I did a search and there are about 15 distribution sites within 20 miles of Cleveland. From the Angel Food Ministries website:

"Angel Food is available in a quantity that can fit into a medium-sized box at $30 per unit. Generally, one unit of food assists in feeding a family of four for about one week or a single senior citizen for almost a month. "

The main thing to know is that everyone qualifies for this! Also, every zip code I could think of in different states had distribution sites. Go to their website above and search for your zip code to get started. You can even view the contents of the different box combos. They have a regular box for families and a senior/convenience box for Seniors or People on the Go!
